Low-Dose Digoxin Reduces Heart Failure Hospitalizations by 25%
Research from UMCG suggests low-dose digoxin can significantly reduce hospitalizations for heart failure patients.

A series of studies led by cardiologists Dirk Jan van Veldhuisen, Kevin Damman, and Peter van der Meer from the University Medical Center Groningen (UMCG) indicates that low-dose digoxin may help reduce hospitalizations and the risk of death in heart failure patients. The findings, published in journals such as Nature Medicine and JAMA, were also presented at the ESC Heart Failure Congress in Barcelona.
Heart failure, a condition where the heart struggles to pump blood effectively, affects over 500,000 people in the Netherlands. Standard treatment involves a combination of four medications, known as the 'Fantastic Four.' The UMCG studies suggest that digoxin could serve as a beneficial fifth medication. In one study involving 1,000 patients across 43 centers, those receiving digoxin saw a 25% reduction in hospital admissions for heart failure compared to those on a placebo.
The studies also found that low-dose digoxin is safe and easy to administer. However, discontinuing digoxin led to increased complications, as observed in a follow-up study of 600 participants. Among them, those who stopped taking digoxin experienced more problems within six weeks than those who never took it.
Digoxin, costing less than ten cents a day, works by reducing harmful compensatory responses in the heart, such as stress hormone levels. Historically, higher doses were used to increase heart muscle contraction, but this was less beneficial. The UMCG research confirms that lower doses are more effective, aligning with earlier suggestions that low-dose digoxin improves patient outcomes.
The research was funded by Hartstichting and ZonMw, which provided 3 million euros through the Good Use of Medicines program. This funding was crucial, as studies on older, inexpensive medications often face financial challenges despite their potential to enhance patient care and reduce costs.
